Development of acute myeloid leukemia during the treatment of non-Hodgkin¡¯s lymphoma

Abstract
The widespread use of intensive combination chemotherapy with/without radiation therapy has enhanced the long-term survival of the cancer patient. However, cytotoxic therapy has led to the emergence of late-complications, such as therapy-related acute myeloid leukemia (t-AML). We experienced a case of t-AML with inv(16) and the subsequent development of the t(9;22) abnormality. This is the first report of the coexistence of these abnormalities in t-AML. A forty-eight year old woman was diagnosed as having t-AML during treatment of low-grade non-Hodgkin¡¯s lymphoma. The patient showed pancytopenia 10 months after the initiation of chemotherapy with a topoisomerase II inhibitor. After the patient achieved complete remission, she received allogeneic hematopoietic stem cell transplantation from a sibling, and has since been in excellent clinical condition. During a follow-up of patients treated for primary malignancies, the possibility of t-AML arising should always be considered. Not only a morphologic evaluation but also a cytogenetic study is necessary to study pancytopenia that may follow chemotherapy.(Korean J Med 72:S322-S326, 2007)
